SPRING GOLF REGISTRATION
Registration opens January 8, 2024!
Any student wanting to participate in sports during the school year must complete all registration through the University High School website via Final Forms
All athletes must complete the online registration, provide a current sports physical and medical insurance prior to participating.
PARTICIPATION IN ALL UHS ATHLETICS REQUIRES:
- ASB Card - ($35)
- Participation/Sport Fee - $70** each season for 1st & 2nd sport in a school year ($60 for District Participation fee & $10 for the General Athletic fee). Fee reduced to $10 for all athlete's 3rd sport in a school year.
- **unless on free or reduced lunch and have signed the Consent to Share Form (Housebill 1660)
- All athletes are expected to pay any fines from prior seasons before starting a new sport.
- A current physical (less than 2 years old) must be on file. This form is available at HERE or at the high school.
- Insurance: All students participating in athletics must be covered by medical insurance
SPORT PHYSICAL:
Your student must have a current sports physical prior to participating in practice or games, no exceptions. Physicals are valid for two years.
