Letter of Recommendation Form (Brag Sheet)

Letters of recommendation are used to help get a holistic view of a student. In order for teachers and counselors to write the best letter of recommendation possible we ask that students complete the "Brag Sheets" below and give a minimum of two weeks notice. These "Brag Sheets" help teachers and counselors to have all pertinent information about the student to write the best letter possible. 

 

 

Please follow-up with your teacher(s) and/or counselor and let them know that you completed the form, when the deadline is for them to get the letter to you and ask if they need anything else.
